Terms & Conditions

      

These Terms and Conditions (“Terms”) govern the services provided by Assurance Medical Testing, www.Assurancemedtesting.com, doing business as Florida Testing Services (“Company,” “we,” “us,” or “our”). By scheduling an appointment, receiving services, or otherwise interacting with our team, you (“Client,” “you,” or “your”) agree to be bound by these Terms.

 

1. Services Provided

Assurance Medical Testing offers the following services:

  • Legal and non-legal DNA testing (paternity, maternity, sibling, avuncular, grandparent, immigration, infidelity)

  • Ink fingerprinting

  • DOT and non-DOT drug testing (urine and hair follicle)

  • DOT-regulated testing for employers

All services are provided by appointment only at approved commercial testing locations. We do not provide in-home or residential drug testing.

 

2. Appointments and Scheduling

  • Appointments must be booked in advance via phone, email, or our website.

  • Same-day appointments may be available but are not guaranteed.

  • Clients are expected to arrive on time. A 10-minute grace period will be honored; late arrivals may result in rescheduling.

  • Cancellations made less than 24 hours in advance or no-shows may be subject to a rescheduling fee.

  • Walk-in clients will only be accommodated based on staff availability.

 

3. Payment Terms

  • Payment is due at the time of service unless otherwise agreed in writing.

  • Accepted forms of payment include credit/debit cards. Cash may be accepted for certain services with prior approval.

  • DNA testing and specialty services require full payment before sample collection.

  • All payments are final. No refunds will be issued once services have been provided or a sample has been collected.

 

4. Legal and Court-Admissible Testing

  • Assurance Medical Testing partners only with accredited laboratories to ensure legal DNA tests meet chain-of-custody and court standards.

  • Clients must inform us at the time of scheduling if results are intended for legal use.

  • Non-legal tests may not be admissible in court.

  • We are not responsible for delays caused by incomplete or inaccurate client information.

 

5. Confidentiality and Privacy

  • All personal information and test results are handled in compliance with HIPAA and industry standards.

  • Results are released only to authorized parties listed on the chain-of-custody or consent form.

  • We will not share test results without written consent, except where required by law.

 

6. Result Delivery Times

  • DNA test results: typically 2–5 business days

  • Hair follicle and lab-based drug tests: typically 2–5 business days

  • Delays may occur due to laboratory processing times, holidays, or incomplete documentation.

 

7. Client Responsibilities

By using our services, you agree to:

  • Provide accurate and truthful personal information

  • Follow instructions provided by our staff and collectors

  • Treat our staff and other clients with respect

  • Refrain from using our services for fraudulent, illegal, or deceptive purposes

 

8. Limitation of Liability

  • Assurance Medical Testing is a collection and testing service provider, not a laboratory.

  • We are not responsible for laboratory delays, inconclusive results, or third-party processing issues.

  • We are not liable for legal outcomes, employment decisions, or other consequences resulting from test results.

  • All results are dependent on the accuracy and integrity of the submitted sample.

 

9. Refusal of Service

We reserve the right to refuse service to individuals who:

  • Are disrespectful, threatening, or disruptive

  • Appear to be under the influence of drugs or alcohol

  • Fail to follow safety protocols

  • Request services outside the scope of what we provide

 

10. Age Restrictions

  • Clients must be 18 years or older to authorize testing.

  • For minors, a parent or legal guardian must provide consent and appropriate documentation.

 

11. Re-Testing and Disputes

  • Requests for re-testing may incur additional fees.

  • If a specimen is rejected due to client error (e.g., insufficient sample, tampering), the client is responsible for all re-collection fees.

  • Any disputes regarding results must be directed to the testing laboratory.

 

12. Governing Law

These Terms are governed by the laws of the State of Florida. Any disputes will be resolved in accordance with applicable state and federal laws.

 

13. Changes to Terms

Assurance Medical Testing reserves the right to update or modify these Terms at any time without prior notice. The most current version will always be posted on our website. Continued use of our services constitutes acceptance of any changes.
